Southside Partnership DLR ?in conjunction with the HSE are now
recruiting for a Community Connector (Social Prescribing for older
people) role. This person will join the Social Prescribing team in
Southside Partnership and will deliver a community connector service
in partnership with the Integrated Care Programme for Older Persons
(ICPOP) in the HSE. The Community Connector role is a pilot project
being implemented in key sites across Ireland. ?The ICPOP Community
Connector service is a specialist social prescribing service for a
defined caseload of older people. The person will work both with
colleagues in Southside Partnership DLR and in the ICPOP team based in
Clonskeagh.
